Blanco Panthers  Pre-Participation Physical Event

TIME: May 24^th, 2023 @3 for BHS Athletics and after school for Band/BMS Athletics/Cheerleading

LOCATION: BHS Gym

COST: $25.00/physical by cash or check payable to Blanco Regional Clinic

The only form which will need to be printed, completed by a physician, and returned to the Athletic Department is the Pre-Participation Physical Evaluation Form. 

To Parent/Guardian and Student:

Blanco I.S.D. will conduct the Annual Preparticipation Physical Evaluation for the 2023-2024 school year on Wednesday, May 24, 2023. All students that will be in 7th-12th grade Athletics/Cheerleading/BHS Marching Band are required to have an annual physical screening before participation.

Any student participating in Athletics/Cheerleading/BHS Marching Band must present a completed physical examination medical history form before participation will be allowed. A failure to do so will result in NO participation or REMOVAL from the activity entirely.  Please do not wait for school or practice to start.

Please read all the fine print on the bottom of the form, fill out the form entirely, and sign the designated box at the bottom of the front page before coming to the physical screening.

LOCATION:  BHS GYM

COST:  $25.00 per student

This is the only time that physicals will be offered by BISD for 2023-2024.
