Physical Evaluation
To Parent/Guardian and Student:

Blanco I.S.D. will conduct the Annual Pre-participation Physical Evaluation for
the 2024-2025 school year on Tuesday, May 14, 2024. All students that will be in
7th-12th grade Athletics/Cheerleading/BHS Marching Band are required to have
an annual physical screening before participation. Click here for the Pre-Participation Physical Evaluation Form.

Any student participating in Athletics/Cheerleading/BHS Marching Band must present a completed physical examination medical history form before participation will be allowed. A failure to do so will result in NO participation or REMOVAL
from the activity entirely. Please do not wait for school or practice to start. Please read all the fine print on the bottom of the form, fill out the form entirely, and sign the designated box at the bottom of the front page before coming to the physical
screening. MUST have a parent signature before physical examination!

TIME: Tuesday, May 14, 2024 (BHS starts 8th period)(BMS After School)
LOCATION: BHS GYM
COST: $25.00 per student

Blanco Athletic Department will use this online platform for all UIL Participation forms. All forms are set to expire annually at the end of the school calendar year. Do not complete until after the Annual Physical Evaluation Event. Physical forms
may be submitted by paper or uploaded directly to Rank OneSport by the guardian.
